T01 Your starting position — Worked example

Hypothetical worked example

Read with chapter 1 to 3

Use this before a first career discussion. Record uncertainty openly; this is not an eligibility assessment.

Current employment

A Specialty Doctor works in a defined inpatient service. The applicable contract and progression route will be confirmed with workforce before any formal application. This example does not select a national route.

Intended destination

Recognition of responsibility already exercised in the regular ward review activity, with exploration of a Specialist SAS role. The doctor is not requesting an unspecified increase in workload.

Present contribution

The doctor leads the agreed ward review and coordinates follow-up with the multidisciplinary team. Existing job-plan wording is generic. A supporting clinician and senior nurse can describe different aspects of the work.

Main gap

Primarily an evidence and documentation gap, with an organisational question about the applicable route. The initial review may identify specific development needs; it should not presume that the whole clinical scope must be learned again.

Next question and people

Ask the clinical director and workforce colleague which process applies and what description of present duties they need. Ask the supporting clinician to review the draft scope and identify the basis of any feedback.

Action and review

Proposed: The doctor prepares T02 before the career meeting; the director identifies the appropriate workforce contact. A review date is to be agreed at that meeting. No grade change or resource commitment has yet been made.
