T03 Capability evidence map — Worked example

2026 digital edition · Dr Naeem Aziz DGM FRCP · 2 min read

Hypothetical worked example

Read with chapter 6

Use one record per selected capability, then assemble an index. Read the exact framework wording alongside the worked method in chapter 6.

Framework and reference

Illustrative mapping to Domain 1, considering SG 1.3 and SG 1.5 in the Specialist framework. Check the exact current wording and its applicability to the claim before using these references in a real application.

Claim

The doctor can explain clinical reasoning and recognise when additional advice is needed within an agreed activity.

Evidence and contribution

CLIN01 records the doctor's reasoning in a hypothetical situation. A completed T06 can record the support arrangement relevant to the activity. A separately obtained account from an observer would corroborate the aspect actually observed. The multidisciplinary team's information and contribution remain explicit.

Limits

One reflective account does not establish consistent performance across the full scope. The example does not provide independent verification, measure patient outcomes or cover all Domain 1 requirements.

Next action

Agree a proportionate review of practice over more than one relevant occasion and seek specific feedback. Use existing ordinary work where possible. Do not collect repetitive patient details to make the record appear larger.

Review judgement

The reviewer should explain what is supported, what remains uncertain and the next useful evidence. The map records that reasoning rather than awarding a traffic-light certification of readiness.
