T04 Supported development plan — Worked example

2026 digital edition · Dr Naeem Aziz DGM FRCP · 2 min read

Hypothetical worked example

Read with chapter 7

Turn an identified need into a feasible opportunity. Agree the review interval locally against the objective, opportunity and evidence needed.

Objective

Develop capability to lead a defined outpatient follow-up clinic. Existing relevant experience is acknowledged; the objective concerns the decisions and responsibilities specific to this clinic.

Opportunity and support

Proposed: agreed clinic sessions with an available supporting clinician, explicit discussion triggers and feedback on reasoning and follow-through. Complete T06 and confirm backup arrangements before the activity starts.

Time and resources

The clinical director must agree how the activity fits the existing plan and what is displaced. Clinical review time also needs recognition. The doctor is not expected to add an unrecorded session to make the plan work.

Evidence

A small set of anonymised educational accounts, specific feedback and the revised role description will be reviewed together. Clinical records remain in approved systems. More independence or a larger caseload is not itself the success measure.

Review

Illustrative proposal: a formal review after six weeks, with earlier discussion if concerns arise. The review examines judgement, communication, scope and the reliability of support. It may retain or increase support as well as agree a change.

Fallback

If staffing prevents the activity, record the missed opportunity and agree another date or alternative. Do not label an unprovided opportunity as the doctor's capability failure. No appointment or regrading outcome is promised.
