T05 Job planning discussion record — Worked example

2026 digital edition · Dr Naeem Aziz DGM FRCP · 2 min read

Hypothetical worked example

Read with chapter 8

Prepare the job-plan request using the worked wording in chapter 8. Separate the applicable contract, professional guidance and your proposed arrangement.

Activity

The doctor proposes a small improvement project concerning the reliability of discharge communication. The need has been identified in the service, but time has not been agreed.

Present arrangement

The doctor has gathered the project outline but has not begun collecting records at home or assumed permission to use clinical data. Existing clinical duties occupy the current timetable.

Request

Agree time for project design, approved data collection, team discussion and review. The request describes the work to be done rather than selecting an arbitrary SPA number.

Effect on work

The director needs to identify which existing activity can move, who covers it and whether the proposed audit support is available. The project scale may need to reduce to fit the resources.

Basis and decision

This is a proposal linked to an agreed service concern. Contractual provisions should be checked separately. In this hypothetical example, no resource decision has yet been made; encouragement alone is not recorded as approval.

Next step

Ask for a named decision owner and response date. If the request cannot be supported, record the reason and discuss a feasible alternative. The unresolved opportunity remains an organisational issue rather than evidence of poor motivation.
