T10 Career meeting and decision record — Worked example

2026 digital edition · Dr Naeem Aziz DGM FRCP · 1 min read

Hypothetical worked example

Read with chapter 10

Use before and after a discussion. Record the actual decision, including pending or unresolved matters.

Purpose

Clarify how prior overseas experience relates to the present role and identify suitable feedback in the new service. The doctor works less than full time. Employment history and formal eligibility require separate confirmation.

Participants

The supporting clinician can discuss practice; the clinical director can consider opportunities; a workforce colleague is needed for contract and experience-calculation questions. Their authorities are recorded separately.

Evidence

A factual employment timeline, T02 scope profile and selected existing evidence are circulated through the agreed system. Overlapping dates and uncertain calculations are labelled for checking.

Discussion

A proposed learning activity falls on a non-working day. The team considers an equivalent opportunity within the doctor's actual pattern rather than treating availability on that day as a measure of commitment.

Recorded outcome

Clinical feedback is requested on the defined current activity. The director will investigate an accessible opportunity. Workforce will clarify how the operative criteria apply to the employment history. No one has yet confirmed eligibility or promised appointment.

Follow-up

Actual owners and dates must be entered when agreed. Circulate a concise record for correction, including unresolved points. A polite conversation should not be converted retrospectively into an approval that was never given.
