Use the tools to support a conversation

The following forms are practical planning aids, not official assessment instruments. They make the reasoning and agreements behind a development or progression discussion visible. Adapt them to the setting and use the applicable national or employer documents for formal decisions. A completed form is not evidence that a decision has been authorised unless the responsible people have actually agreed it.

Each blank form is followed by a hypothetical example. Use or reproduce the blank forms for individual professional purposes. Where more space is needed, continue on additional pages or in an expanded electronic copy. The examples illustrate the level of specificity useful in a discussion. They are not signed agreements or evidence from real applicants. In your own record, replace placeholders with accurate details and distinguish proposals from confirmed arrangements. Dates and contacts matter once an agreement is made.

Keep patient information out of these forms wherever possible. Use authorised systems for personal or employment information and agree who can see the records. A portfolio copy may need different handling from the source record. The forms are not clinical records and do not replace them.

Use only the tools that help answer your present question. T01, T02 and T10 are often sufficient for a first meeting. Add T03 and T04 when the capability discussion becomes specific. T05 and T06 support decisions about work and supervision. T07 to T09 help make learning and contribution visible. T11 and T12 belong to the application and response stages. This sequence is a convenience, not a compulsory pathway.
