T11 Application preparation review — Worked example

2026 digital edition · Dr Naeem Aziz DGM FRCP · 2 min read

Hypothetical worked example

Read with chapter 17

Check completeness against the real process. This review is not a prediction of success and does not replace official forms.

Route

The doctor has assembled material to explore Specialist progression. The preparation review begins by confirming the applicable process; the example does not assume that every UK doctor can use a national regrading route.

Eligibility and scope

Workforce confirmation of the operative conditions is retained with the factual timeline. The current activity is described consistently across T02 and the role documents. Any disputed description is identified rather than silently replaced.

Evidence

The index links claims to selected records and explains their limits. Relevant clinical feedback and broader professional contribution are included. Course certificates support learning history but are not presented as proof of changed practice on their own.

Verification

Check that each statement of observation has a real basis and that no team result is claimed as an individual achievement. Remove unnecessary identifying information from the submission copy using the approved process.

Unresolved matter

If the route or a required form remains uncertain, obtain clarification before presenting the evidence pack as a completed application. Record the responsible contact and response date.

Review conclusion

The review can conclude that the documents are ready for submission under the confirmed process. It cannot declare that a panel must appoint or regrade the doctor. Those decisions belong to the authorised process.
