Specialist registration, Portfolio Pathway and your career choices

Four things that are often confused

A Certificate of Completion of Training (CCT) follows completion of a GMC-approved UK training programme. A Certificate of Eligibility for Specialist Registration (CESR) can be awarded after a successful Portfolio Pathway application demonstrating the knowledge, skills and experience required by the current UK standard. Both can lead to entry on the Specialist Register, but the training history and application route differ.

Specialist grade is a contractual SAS employment grade introduced under national SAS arrangements. It does not itself put a doctor on the Specialist Register. Likewise, Specialist Register entry does not automatically change a current contract, confer a consultant post or require an employer to create one.

• Regulatory status and employment grade are separate.
• A successful Portfolio Pathway application currently results in a CESR.
• Job titles do not prove register status.

What the Portfolio Pathway tests

The GMC asks whether documentary evidence demonstrates the required knowledge, skills and experience for the relevant specialty. The assessment concerns knowledge, skills and experience against the current specialty requirements; long service, seniority, loyalty or carrying a heavy rota does not by itself demonstrate those requirements.

The assessment is documentary. Evaluators cannot fill gaps by assuming that an experienced doctor probably has a capability. Every required outcome must be evidenced at the appropriate level; exceptional strength in one domain does not cancel a missing domain elsewhere.

• Demonstrate current performance, not only historic exposure.
• Do not rely on reputation or job title.

Registration and consultant appointment

Specialist Register entry normally enables a doctor to be considered for substantive consultant appointments in the relevant specialty, subject to the appointment rules and employer process. It does not guarantee shortlisting, appointment, a particular job plan or pay outcome. Employers assess the advertised role, person specification, values and competition separately.

Some doctors seek registration to widen future options, strengthen professional identity, pursue independent practice where relevant or contribute at a different level. Others decide that Specialist grade, portfolio work, education, leadership or a sustainable Specialty Doctor role better matches their goals.

• Treat registration as an enabling status, not a job offer.
• Plan the post-registration transition before submission.
• Keep multiple career destinations open.

A senior SAS career is a valid destination

A Portfolio Pathway decision should not imply that SAS work is second best. SAS doctors can practise with substantial autonomy, lead services, teach, research and hold organisational roles. Specialist grade can recognise a defined senior scope without Specialist Register entry, subject to national arrangements and employer need.

The better question is not ‘Am I ambitious enough?’ but ‘Which destination fits my desired scope, responsibilities, evidence access and life?’ A decision not to apply now can be reviewed later; a decision to apply should be supported by a realistic plan.

• Separate status from professional worth.
• Include wellbeing and family impact in the decision.
• Review the decision when circumstances change.

A practical decision test

Clarify the desired clinical scope, leadership responsibility, income expectations, geographical flexibility and time horizon. Then identify whether Specialist Register entry is necessary, helpful or unrelated to each goal. Estimate evidence gaps and what the employer can realistically provide.

Discuss the plan with a knowledgeable specialty adviser, College or Faculty contact, SAS tutor or deanery service where available, and a manager who can address work opportunities. Each person has a different role; supportive enthusiasm is not the same as a formal assessment of readiness.

• Define the destination before the route.
• Ask specialty-specific advisers about the current standard.
• Record assumptions and review dates.

Use current terminology and dated requirements

The GMC renamed the CESR/CEGPR application route the Portfolio Pathway on 30 November 2023. Successful specialist applications still lead to a CESR and Specialist Register entry. Older College, employer and deanery pages may retain historic wording.

Terminology, forms, curricula, evidence policies and fees change. The live GMC application guide, current specialty-specific guidance and GMC Online requirements at the point of submission take priority over this independent programme or an older teaching session.

• Reconfirm requirements immediately before submission.
